No Death Anchor set on death

- Barbara attacked you with a Tailwhip and hit for 12 points of slashing damage. Your aura of entropy decays them! They take 1 points of death damage! This was enough to kill you! (2022-10-04 09:39:15).

We were flying at the time of the death.

This is unfortunately uninvestigatable - some tile types like void don't allow Death Anchors, and it's possible you were over one of those.
