The issue was you rolled a death roll on a NPC Pariah who was already in the void (due to lag or simultaneous actions), but the notification method is set to notify whoever is logged in and triggering the effect.

It’s kind of obscure, but there’s a pending fix for it. It’ll be in the next patch - thanks for flagging it for us.
