Explosive Murder on Death Never Hits Anything

Explosive murder (the upgrade) on character death explodes, but always says 0 characters are affected.
Unsure if this affects intentional EM or not.
It is definitely not zerg flag.
Could be an out-of-order operations thing, where the AoE takes place in the death plane instead of the mortal planes?

Have been hit by Explosive Murder a couple times, can't replicate.

Here's logs for Liberia since purchasing EM.
9 deaths to pets with 0 reported explosions.
4 deaths to characters with 0 reported explosions.
Some of those pet deaths involved several pets on the same tile (e.g. feral hellhound packs, skeleton hordes).

Priseth reports not seeing any explosion happen on her end in the logs.
This suggests that the death plane explosion theory might be valid.

Also, manually triggering EM works as intended.
